Call Pepper FIRST to book a spot (510) 548-6128. Also, please fill out a form for EACH EVENT you intend to participate in.
Forms must be received by May 15, 2024 or we will have to drop you from the show. Thank you.
Please download the form BEFORE filling it out (open with Acrobat Reader).
Print and fill out the form, then MAIL along with payment to:
CARNIVAL OF STARS, 1115 Bancroft Way, Berkeley CA, 94702.
Only the Troupe Leader signature is needed on each returned form.
Everyone is required to sign the form at Check-In on day of performance.
