PLEASE NOTE INSTRUCTIONS on each form.
Call Pepper FIRST to book a spot (510) 548-6128. Also, please fill out and send in a form for EACH EVENT you intend to participate in.
Forms must be received by May 1, 2026 or we will have to drop you from the show. Thank you.
Please download and print the form BEFORE filling it out (open with Acrobat Reader). MAIL the completed form along with payment to:
CARNIVAL OF STARS, 1115 Bancroft Way, Berkeley CA, 94702.
For DANCER REGISTRATION: only the Soloist or Troupe Leader signature is required when submitting the form.
Everyone is required to sign the form at Check-In on day of performance.
